Shape. The surface has to be crowned so water leaves it. Flat means water sits, the subgrade softens and traffic works it into ruts. It is free to do at build time and expensive to fix later.
Drainage that goes somewhere. Crown moves water to the edge, ditches carry it away, culverts take it under the road where a draw crosses. All three have to work together. A ditch that dead ends is not drainage.
A subgrade that was actually built. Stripping the topsoil and compacting what is underneath is the step that gets skipped, because nobody can see it once the job is finished. It is also the step that decides whether the road holds.
